The Dementia Care Plan can slow progression of the disease and postpone placement in an assisted living facility.
Dementia Care Plan
The dementia care plan will assist the client and their family in attaining the highest quality of life given their diagnosis of Alzheimer's disease or dementia.
Age @ Home will create, implement, and monitor the dementia care plan to ensure that it continually meets the needs and desires of the client and their family.
