Elder Care Management Services     

Case Study:  Mrs. M is 79 years old and lives alone in her own home.  Her adult daughter who lives in San Francisco,is concerned about mom who is increasingly confused and forgetful.  On her last visit to San Diego she took mother to the doctor and he suspects some form of dementia.  The doctor wanted a full work-up and reccomended that the daughter who has a demanding career, work with Age @ Home care manager, Kathy Randall.  After the assessment which included a cognitive evaluation, Ms Randall set up and accompanied Mrs. M to the neurological evaluation. The neurologist diagnosed  Alzheimer's and started medications to slow the progression of the disease.  Care Manager also reccomended some caregiving services for cooking, light housekeeping and running errands.  Care Manager made sure the advance health care directives and durable power of attorney were up to date and in place.  Mrs. M is now safely at home with some support and socialization. She attends a weekly early stage Alzheimer's support group.  Daughter reports her phone conversations with her mother are more upbeat. The care manager continues to monitor the situation and communicates with the daughter on going.
